Is there a "best format" and what is the maximum resolution allowed...or is there only one resolution?

Format - For compatibility with all versions of the game (windows, mac, linux), you should use the .mov container with a compatible encoding format and profile. I believe that Windows is the most restricted platform, so if your file plays back on windows it should play back on the others too.
Here is an example of the format info for the epilogue cutscene:
