Sounds like something is messing up when it's supposed to play the cutscene.

As a separate note, any reason you're using Software rendering? Try OpenGL, it should run and look a lot better for you.

Do you have anything in your local folder?
That stack trace says that Myth crashed because it was trying to load a bad collection...

I tried using software rendering to see if it worked. It was at OpenGL originally.
There is nothing in the Local folder.

It looks like it's crashing when loading a saved game. Can you zip up and post the saved game file for us to look at?
Also, the log indicates that it's crashing during loading a game - which seems inconsistent with your mention of crashing when SB falls into lava. Unless perhaps Myth is trying to load a new game after this, and failing.
For sanity, can you post MD5 sums of the files inside your tags folder?
You can do this using this utility:

Here you go. Hope I did the MD5 right. This save game file, it's the only one I've been using and saving to after every level.
I can also start a new game and play the last level to see how that goes.

Your tags look correct.
Can you do the same (MD5) on all the patches in your plugins folder? (i.e. all the ones that Myth is loading per your myth_log.txt)
Also, does this only happen on Forge?

Yes, it's only happening on Forge. Here's everything in that folder.

Can't really see anything wrong with those either...
Does it crash right as SB is falling into lava? Or does the game end and the epilogue cutscene and journal text are played and it crashes after?
One more thing to try: start a new game on the Forge and press Ctrl + (plus on the keypad) to "win" the level. Does it crash when doing that?

Right after SB falls into the lava. I don't see any cut scene or journal.
When I press CTRL +, they all cheer then it crashes.

Download this
zip file and extract. It contains all the Myth2 executable files for Windows from 1.3 to 1.7.2. Place 1.7.1 into your Myth II folder and run it. Go to The Forge and use Ctrl + to beat it. Does it crash with that version too? If it still does the same, try an older one like 1.7.0. If we can find a version that does not crash then we might be able to figure out why it crashes and the devs can fix it.
If 1.7.1 does not crash, then try the 1.7.2 build from that zip as well. This is to see if that build is somehow different from your build.
